Pease note that:

Depending on your identified health goal and identified imbalances there might be variable levels of benefit obtainable from the Nutritional Therapy consultations.

Personalised Nutritional Therapy advice is designed to support health conditions and/or health concerns/imbalances identified and agreed between both parties.

Any health advice, discussions provided via general marketing media such as videos/blogs/recipes is not personalised.

Nutritional Therapists do not diagnose or treat medical conditions.

Nutritional advice is not a substitute for professional medical advice and/or treatment.

Standards of professional practice in Nutritional Therapy are governed by the BANT Code of Professional Practice.

Former clients, who have not participated in a one to one consultation longer than six months are classed as ‘former clients’. Please note all supplement and food plans are generally only relevant for six months post consultation.

The Client Agrees to the Following:

You are responsible for contacting your GP about any health concerns.

You give permission to Daisy Ilchovska (NT) to contact your GP regarding any agreed aspects of your case.

Agreement to client referral will be taken as consent to disclose information to the relevant 3rd party only.

If you are receiving treatment from your GP, or any other medical provider, you should tell him/her about any nutritional strategy provided by a nutritional therapist. This is necessary because of any possible reaction between medication and the nutritional programme recommended to you.

It is important that you tell Daisy Ilchovska (NT) about any medical diagnosis, medication, herbal medicine, or food supplements, you are taking as this may affect the nutritional programme.

If you are unclear about the agreed nutritional therapy programme/food supplement doses/time period, you should contact Daisy Ilchovska (NT) promptly for clarification.

You must contact Daisy Ilchovska (NT) should you wish to continue any specified supplement programme for longer than the original agreed period, to avoid any potential adverse reactions.

You are advised to report any concerns about Nutritional Therapy promptly to Daisy Ilchovska (NT) for discussion and action.

Recording consultations using any form of electronic media is not allowed without the written permission of both the Nutritional Therapist and the Client.

It is essential that all health questionnaire and other forms are returned by email, an absolute minimum of 3 working days before the consultation. You understand that your completed health questionnaire will be used as a basis for our first consultation. All first consultations are necessary for any nutritional therapy programme to begin and will last between 1 and 1 ½ hours.

We take payment in advance for the consultation package by credit/or debit card.

As most appointments are arranged back to back throughout the day, please ensure that you arrive 15 minutes early, ready for your allocated appointment time. If you are late then we will use the remaining time of your appointment to examine your case.

Please be advised that we operate a 72 hour cancellation policy (excluding weekends) and if you fail to notify us within this time period a full fee for the appointment will be charged.

Clients are advised that OH Nutrition recommends the highest quality supplements, however, sometimes, for varying reasons, clients have had reactions to products, which is out of our control. In such rare occasions clients are advised to contact us immediately.

It is the client’s responsibility to inform Daisy Ilchovska (NT) if, at any point during their treatment programme, they are having an operation or any type of surgery.
